Nutritional Assessment Questionnaire – $295

With this service, you fill out a few electronic questionnaires that summarize your symptoms and health goals. After it’s submitted, I study it and create an overall protocol that is completely bio-individual to you. I take into account your health issues and your health goals to help you begin your journey towards healing.
What You Receive:
- A protocol created by me for you that includes nutritional recommendations, lifestyle recommendations, and supplement recommendations that are personalized to you.
- Information packets that include grocery recommendations and nutrition / lifestyle education.
- You’ll have a client account with access to a dispensary of medical-grade supplements with an exclusive client discount. The discount won’t ever expire.
1:1 Client – $1995
Need more intensive nutritional therapy? Would you like support on your journey of healing with check-ups and plan adaptations as you go? With this service, you will have a 4-month 1:1 client plan with individualized support.
As a 1:1 client, you will receive:
- A Nutritional Assessment Questionnaire and Initial Intake call on zoom or phone.
- 8 one-hour follow ups over phone or zoom.
- Unlimited text message and email support during business hours.
- Individualized protocols created for you, including nutrition, lifestyle, and supplement recommendations that will be adapted throughout your entire program as we work through your health concerns.
- Information packets that include grocery recommendations and nutrition / lifestyle education.
- You’ll have a client account with access to a dispensary of medical-grade supplements with an exclusive client discount. Your discount will still apply even after the plan is over.
